FORMS and OTHER SERVICES AVAILABLE AT COA
There are many forms and other items which we have at the Senior Center to assist you. Unless a price is stated - it is free. Listed in alphabetical order:
Adaptive Phone Equipment free of charge to people with disabilities. Completed application must be submitted to and be approved by Verizon. Approval by Primary Care Physician is also required.
Cell Phones capable of reaching 911 in an emergency.
Disabled Placard/Plates Applications are available for any disabled Massachusetts resident. Completed application must be sent to Medical Affairs Branch of the Registry of Motor Vehicles
Enhanced 911 – Disability Indicator Information required to complete this form if you want the Police Dept., Fire Dept., or other emergency agency to know about your circumstances when you call 911.
File of Life is a magnetized pouch which adheres to your refrigerator. The pouch contains a form with important information needed if an emergency occurs.
Health Care Proxy is available for you to choose your health care agent and treatment. There are many choices in life support, it is important that your family know your wishes.
Homestead Protection Forms are available for those 62+ ($200,000/person) and under 62 for $100,000 on the home. These protect you from losing your home due to a lawsuit. You must be the owner of the home to file a homestead for which there is a fee.
PVTA ID Card is available for those 60+ or handicapped residents who qualify within PVTA guidelines and have physician certification. This card qualifies the holder for reduced transportation costs throughout Mass and is good for 5 years; cost $1. Check calendar for days camera is here to take pictures.
