Required Parent Paperwork

Camper Information Form: A camper information form must be filled out for all campers. This will help us get to know your child better and make for a smooth summer at camp. If you have completed the online registration process, this step has already been completed. If you completed registration at the Recreation Office, you will receive a paper form for this information.

Emergency Contact and Medical Information Form: This upcoming summer we will be piloting a new system called ePact. This will allow parents to fill out emergency contact and the medical form online. This will be easily accessible to all staff with XTAC in the event of an emergency. This will also allow easier communication between parents and XTAC staff.

Immunization Records: The Town of East Longmeadow only operates licensed camps which follows regulations provided by the State of Massachusetts. One of these regulations requires all campers to be up-to-date with immunizations, and that a copy of immunization records be turned in prior to the start of camp. You will be required to turn in a physician issued copy of up-to-date immunization records to the Recreation Office prior to the start of camp.

Parent Handbook and Session Calendars

Information regarding the parent handbook and session calendars will be uploaded by early June, and will be sent to program registrants.
